Brand claims such as "Doctor-formulated" and "FDA-approved facility" appear in the sales copy. Neither names a person, a company or a site. FDA registers facilities and does not approve them (PolitiFact 2025, 'FDA registered'), so the second phrase is not an approval.
The brand name "ProDentim" is the only name on the label front. A brand name tells you what the product is called. It does not tell you who made it, where or under what quality checks.

What should a distributor line show on a supplement?
It should show the name and place of business of the maker, packer or distributor. FDA labeling rules call for it on a supplement (21 CFR 101.36 (eCFR)).
It often reads "Manufactured for" or "Distributed by", with a street address. That line tells you who answers for the product if something goes wrong.
