Oral Probiotics Explained: What They Are and What Studies Tested
Oral probiotics are live microbes taken by mouth with the aim of supporting the mouth rather than the gut. Effects belong to specific strains, and trials so far are small and use lozenges or dairy more than tablets. ProDentim names three strains, but no trial of the tablet itself was found.

Definition01
What are oral probiotics?
Oral probiotics are live microbes taken by mouth in the hope of helping the mouth, teeth, gums or breath. The word itself comes from the general idea of a probiotic.
ISAPP defines a probiotic as a live microbe that gives a health benefit when taken in enough amount (Hill et al. 2014, ISAPP consensus). The NIH adds that many products sold as probiotics lack clear proof for the claims on them (NIH ODS, Probiotics (HP)).
"Oral" describes the target, not a new kind of microbe. Many strains used in the mouth are the same species used for the gut, such as Lactobacillus and Bifidobacterium.
What makes a product oral is the form and the aim. Lozenges, chewables and soft tablets are made to be sucked or chewed so the strain meets the mouth first.
Strains02
Why does the strain matter in an oral probiotic?
The strain matters because probiotic effects are strain-specific. A result for one strain does not carry over to another, even in the same species.
A species is like a dog breed group, and a strain is one named dog. A strain code, such as the BL-04 on ProDentim's sales page, tells you which one was meant. Species alone does not (Hill et al. 2014, ISAPP consensus).
The sales page names Lactobacillus paracasei, Lactobacillus reuteri and B. lactis BL-04 (ProDentim sales page). Only BL-04 has a code. For the other two, we cannot match the tablet to any trial.
Tap each bubble below to see what is known about that strain, and what is not.

B. lactis BL-04Bifidobacterium animalis subsp. lactis, strain BL-04Amount: not printed
The sales page says 3.5 billion for the three together and gives no split or strain codes except BL-04. The round bacteria pictures are brand images; none says which strain it shows (ProDentim sales page).
Research03
What have studies of oral probiotics tested?
Studies have tested a few strains in lozenges, milk and chewables on plaque, gum scores and decay bacteria. Results are small and mixed.
A review of gum disease trials saw small gains when a probiotic was added to a dentist's cleaning (Benavides-Reyes 2025, probiotics & perio). Reviews on tooth decay call the evidence limited (SR 2025, probiotics & caries prevention) (Gruner 2016, probiotics caries & perio).
Some trials used L. reuteri lozenges (Schlagenhauf 2016, L. reuteri lozenges) (Liu 2025, L. reuteri oral health review), and another looked at L. paracasei and decay bacteria (Chuang 2011, L. paracasei GMNL-33 null). None tested ProDentim.
| Area | What trials looked at | Limit |
|---|---|---|
| Gums | Gum scores after cleaning, with a probiotic added | Small trials; other strains and forms |
| Plaque | Plaque scores with two strains | Short trials; not the ProDentim tablet |
| Decay bacteria | Counts of decay-linked bacteria | Counts are not cavities |
| Breath | Odor measures | Mixed results; see the breath review |
Forms04
Is a soft tablet the same as a lozenge or a capsule?
No. A soft tablet, a lozenge and a capsule release a strain in different ways and in different places. A result for one form does not apply to the others.
A lozenge stays in the mouth for minutes. A capsule is swallowed whole and meets the gut. A soft tablet that you chew slowly sits somewhere in between.
ProDentim is labeled "30 Soft Tablets", and the sales page says to slowly chew one every morning. The page also calls it a "dissolvable candy" (ProDentim sales page). The label front says 'Soft Tablets'. The sales page says 'dissolvable candy' in its FAQ and 'tablet' elsewhere, and tells you to chew slowly. Neither page says capsule, gummy, drops or powder.
Pictures of round bacteria on the brand's pages do not say which strain they show. They are art, not data.

Are oral probiotics safe for everyone?
No. Live probiotics are well tolerated by most healthy adults, but not everyone. A few people have had serious infections.
The FDA has warned about use in preterm infants (FDA 2023, probiotics in preterm infants). Reports also exist of rare infections in people with weak immunity (Costa 2018, probiotic infections SR).
NIH ODS and NCCIH advise asking a clinician first if you take antibiotics or have a weak immune system (NIH ODS, Probiotics (HP)) (NCCIH, Probiotics: Usefulness & Safety). Never stop a prescribed medicine because you started a supplement.
- Pregnant, nursing or under 18: ask first.
- Dental work planned or gums that bleed: ask your dentist.
- Allergies: the panel on your tub is the only source we cannot check here.

Where do oral probiotics fit in a daily routine?
They fit after the basics, not in place of them. Brush twice a day with fluoride toothpaste, clean between your teeth daily and see your dentist.
The ADA points to those habits as the base of oral care (ADA MouthHealthy, Brushing Your Teeth) (ADA MouthHealthy, Flossing). Probiotics are an optional extra, and no source we found says they replace any of the three.
If you try one, keep a note. Write down the start date, the dose on your own tub and what you notice at your next dental visit. That gives your dentist something real to work with.
Give any change time to show. A dentist can compare plaque, gum color and bleeding between visits, which a mirror at home cannot do.
More on the habits in the guide to brushing, flossing and your dentist.

How does ProDentim fit this picture?
ProDentim is one example of an oral probiotic product: a dietary supplement with three named strains and a four-item blend.
Its strengths are a clear form and count on the label, strains named on the sales page, and a one-a-day routine. Its gaps are missing amounts, no per-strain count and no product trial.

Oral probiotics questions
Are oral probiotics the same as gut probiotics?
Not always. Some species overlap, but oral products aim at the mouth through lozenges, chewables or tablets. A result for a gut product does not carry over, because effects depend on the strain and the form (NIH ODS, Probiotics (HP)).
Which oral probiotic strain is best?
No source we read names a best strain. Trials tested a few, such as L. reuteri and S. salivarius M18, with small and mixed results (Schlagenhauf 2016, L. reuteri lozenges) (S. salivarius M18 lozenges, kids 2023). Ask your dentist before you pick one.
Do oral probiotics change the bacteria in your mouth?
Some trials report short-term changes in certain bacteria while the product is used. Whether that helps teeth or gums over time is not settled, and none tested ProDentim.
Can children take oral probiotics?
Ask a pediatric dentist or doctor first. The FDA has warned about preterm infants (FDA 2023, probiotics in preterm infants), and the brand site gives no guidance for children in the images we have. We cannot confirm a safe amount.
How long do you take an oral probiotic?
Trials vary from days to months, and no standard exists. The ProDentim sales page says one tablet each morning but gives no end date. Review any new product with your dentist after a few weeks.
Do oral probiotics replace mouthwash?
No source says so. Mouthwash and probiotics do different jobs, and some rinses affect live microbes. Ask your dentist which rinse suits you if you start a probiotic.
